What if I’m uninsured?
SB&H does not discriminate against any person receiving care because of his/her ability to pay for the services or because payments for those services are made under Title XVIII (Medicare), Title XIX (Medicaid/AHCCCS) or CHIP, or for reasons of race, color, sex, national origin, age, sexual orientation, gender identity, disability or religion. No one will be denied access to services due to inability to pay. We offer integrated health care services on a sliding fee discount for patients based on family size and income. This may include a nominal charge or no charge. We accept assignment under the Social Security Act for all services for which payment may be made under Part B of Title XVIII (Medicare) and Title XIX (Medicaid/AHCCCS).

SB&H Facts
- Incorporated in 1974 as a 501(c)3 nonprofit organization.
- We serve the greater Phoenix metropolitan area, Payson, Buckeye, Flagstaff, Kingman, Prescott Valley, Bullhead City and Lake Havasu City.
- Provide behavioral health services to approximately 9,000 individuals per month.
- Outpatient services provided in 16 clinics located throughout the state of Arizona.
- Provide therapy for opioid dependence in 4 clinics throughout the state.
- Provide counseling and psychiatric services for depression, anxiety disorders, substance abuse, relationship and domestic issues.
- Operate 2 inpatient units for persons with acute psychosis or depression and are a danger to self or others (+2,000 patients in 2014). A third inpatient is scheduled to open in Kingman in early 2017.
- Provide long-term psychiatric care for the infirmed, at nursing homes and assisted care facilities.
- Founding partner of the Southwest Network in 2001.
- SB&H Training Institute’s Doctoral Psychology Internship Program is accredited by the American Psychological Association.
- Research partnerships/collaborations with University of Wisconsin, Northeastern University, University of Rhode Island, Arizona State University, Argosy University, and Glendale Community College.
- Began 3-way value based contracting with Health Choice Integrated Care in 2016.
- Our Community Services Prevention Program served 474,772 persons (direct & indirect) in 2015.
- Provide prevention and early intervention services in more than 45 schools in Maricopa County.
- Provide housing for more than 375 persons with serious mental illness.
- Placed more than 26 Masters level interns and 15 post doctoral interns and residents in SB&H programs in FY2016.
- Largest provider of housing and support services for persons with HIV (HOPWA).
- Currently employ more than 900 staff.
